Because Source Features are points, lines, and polygons, the entire SF set has to be exported into 3 separate shapefiles/gdbs. Attached is a file including the queries which include the Visits and References data that can be run in Query Builder to export as a Shapefile or Geodatabase.  You could also make a view out of these to make it a little easier to query. The data is separated with a | character which could be changed to another character. 

NOTE: shapefile data is truncated after 254 characters, so to avoid any truncation, export to Geodatabase.
